NHS England require all service providers to ask all patients to complete a small survey called the 'Friends and Family Test Initiative'. This is a simple survey that records how satisfied every patient is with the service received and asks all patients to rate how likely, or unlikley, it would be for them to recommend the service to their own friends and family.  The responses can be completely anonymous, but is an essential component in assessing how well we are delivering dental services.  Responses feed directly into NHS England and will help in ensuring you receive services of the highest standards.  It would be greatly appreciated if all patients would complete a survey form, which are either given out at the end of treatment in surgery, or kept in the reception area.  Once completed all forms need to be placed into the Friends and Family Test postbox in the reception area.
